A physical examination must be completed on or after April 15, 2023 to be considered a valid physical for 2023-24. All students who participate on an interscholastic team must have a physical examination completed prior to participation each year. Visit the Ascension Health Center in our building for a no-cost physical. It is open all summer! Call 586-576-4038 for an appointment.
The Participation Contact & the MHSSA Physical Form must be completed and handed to the Athletic Department (or Coach) prior to tryouts.
CLHS Ascension School-Based Health Clinic
CLPS, in partnership with Ascension Southeast Michigan Community Health, received a federal grant to create a school-based health clinic in Center Line High School. In this space, grant-funded staff from Ascension will provide free direct mental health and medical services to all students living in the district, whether or not they attend our schools, including athletics physicals.
